With two of my vehicles, a 2000 Dodge Dakota and a 2011 Toyota Prius, they won't let me lock the filler nozzle while gassing up, under some conditions. It seems like a back pressure condition. Once I get a few gallons in by delivering with a lower flow, then I can engage the latching device and it fills OK.
I never fill unattended.

I'm from PA myself, the Pittsburgh area an I still work there even though I live in Eastern Ohio myself. The little town I'm in, I get gas from $2.68 currently and they pump it for you too. Glad it's cheaper, I drive 90 miles round trip to work.

The filler pipe on my '59 Cadillac is maybe 12" long. Can't do the automatic or I shoot about a pint before the automatic catches up. Even by hand and listening I still fail. With the dual exhausts right over the spill I roll the car in neutral away a bit before starting. I find stations with a slight downhill tilt.

I worked as maintenance for a gas station for 5 yrs once, SEEN IT ALL.... Dumped ALOT of absorbent to clean it up as well.. Changed MANY hoses and quick disconnects.
Worse was skimming diesel off the water when a truck driver capped his nozzle..
