Then you would not turn the switch all the way to OFF. The positions on the switch were OFF, ACCessory, RUN, START. A spring would return the key from START to RUN when you released the key.
On even earlier cars (like 1930s to early 1950s, I guess - before my time) the starter motor was activated with a foot switch.
