In the Oelrich publications service invoice books, on one of the duplicate pages there was some kind of a release page, that the customer was supposed to sign removing the servicer from the liability of the CRT, in case of implosion. That was at the time of CRT replacement.
I never made one out, as it was some form of legalese???.
