I call that the dreaded "Oh No!" dot.

Is it caused by inrushing air (i.e., from broken neck) blowing phosphor off the face?

I recently saw the "Oh No!" dot on a 50/51 Zenith. I was very disappointed as it was the first one i've come across in person.

I bet they said a hell of a lot more than "Oh No" when it first happened!
