I hope someone has a Simpson-Sears directory for those numbers. That TV was made either by Toshiba or Sanyo but the numbers don't cross to anything sold via Sears of Chicago.
Simple parts in them either way, so it is certainly worth the effort. Based on Sears and Roebuck, the Sams Photofact index lists the numbers as 528.xxxxx for Warwick sets and 562.xxxxx and 564.xxxxx, 456.xxxxx for the Japanese manufacturers

Sears tv identification
It is a Canadian set I believe
As for the repairs it has no high voltage and the damper tube is bad, probably also needs a recap. Not sure of the crt as I don’t have a tester. Other than that I’m open to any and all comments.
